The following documents are what are required of each student participating in Athletics each school year. Every document must be completed in full and returned to the appropriate coach prior to the athlete participating in any practice, scrimmage, tournament, or game.

Click here for the GISD Athletic Participation Physical Examination Packet. These 4 pages must be filled out entirely and signed in all the appropriate places by both the student and the parent/guardian. Also included in this packet is the doctor's portion of the UIL Physical Examination. The originals of all 4 documents will be kept in the respective Athletic Trainer's office. Click here for the GISD Student Athlete Code of Conduct Handbook. This handbook includes a signature page that must be filled out and signed by both the student athlete and the parent/guardian. The signature page must be returned to the head coach of the sport in which the athlete is participating in.
Click here for the Extra-Curricular Random Drug Testing Form. This form must be filled out entirely and signed by both the student and the parent/guardian. This form is to be returned to the head coach/sponsor of the extra-curricular activity that the student is participating in.
