GISD HOLDING ATHLETIC PHYSICAL EXAMS ON AUGUST 1

Students who will participate in Granbury ISD athletic programs during the 2012-13 school year will have an opportunity to get their mandatory physical examinations during a session sponsored by Lake Granbury Medical Center on Wednesday, August 1, 2012, from 2:00-6:00 P.M. at Granbury Middle School.

The exams are open to all Granbury students in grades 7-12. Cost for the exam is $20 cash.
 
Athletic physicals are required by the University Interscholastic League (UIL) and GISD and must be completed before a student may participate in any practices or games.
 
This event is not mandatory, and all parents and students have the option of completing this requirement at the medical provider of their own choice – a physician, a physician assistant licensed by the State Board of Physician Assistant Examiners, a registered nurse as an advanced practice nurse by the Board of Nurse Examiners, or a doctor of chiropractic.

Athletic physical packets must be completed by the parent or guardian prior to the examination. Forms are available at Granbury High School, Crossland Ninth Grade Center, AMS, GMS, both training rooms at the high school, and the GISD athletic office. The forms may also be found online on the GISD website at www.granburyisd.org/piratesportsmedicine.
 
Athletic trainers will collect all completed forms at the conclusion of the exams and will be kept on file. Parents may request copies if needed, allowing 48-72 hours for processing.
