The Previous Athletic Participation Form must be completed when a student athlete registers with GISD to play athletics. This form must be completed and signed by all appropriate parties BEFORE the student can participate in any games or contests. Once the student and parent/guardian has completed their portion, the form should be delivered to the GISD Athletic Department for review and submitted to the appropriate parties.  The head coach will be notified when the student athlete has been cleared by the District Executive Chairperson.
