    Objective of Event:

    Athletic Physical Days will provide an opportunity for the GISD student athletes to complete their annual physical examination and have their physical on file with the GISD Athletic Trainers prior to the first day of practice or school. This event is not mandatory and all student athletes have the option of completing this requirement at the medical health care provider (Physician, Physician Assistant licensed by a State Board of Physician Assistant Examiners, Registered Nurse recognized as an Advanced Practice Nurse by the Board of Nurse Examiners, or Doctor of Chiropractic) of their own choice, however, the Athletic Physical Days will allow an inexpensive, quick and easy way to complete the required athletic physical before the summer break. Additional paperwork for the GISD Athletic Participation Packet may be required to be completed online at a later date, this information will be announced when it is available.

    1. The medical history (the left-hand column) on the Athletic Physical Form must be filled out prior to Athletic Physical Please provide explanations to all questions answered “YES”.
    2. The parent/guardian must sign the bottom of the Athletic Travel Form on the back side of the Athletic Physical Form providing consent for the student athlete to receive a physical exam, even if the student athlete is 18 years old.
    Without these two requirements completed, the student athlete will not be able to receive a physical on Athletic Physical Day until resolved.


    Failure to do so may result in delaying the clearance of the vision portion of the exam 

    As per UIL and GISD rules and regulations, all student athletes, student managers (including statisticians, video filmers, class aids, etc.), student athletic trainers, transfer students, and foreign exchange students must have a completed 2021-2022 GISD Athletic Physical on file in the Athletic Trainer’s office prior to any participation in practices, before, during, or after school (both in-season and out-of-season), or game/matches.  Failure to do so will result in the student athlete not being allowed to participate in any GISD athletic sports.  The GISD Athletic Physical Form will be valid from April 1, 2021 through May 31, 2022.

    The GISD Licensed Athletic Trainers will be collecting all completed physical forms from the student athlete at the conclusion of their physical examination to file in their respective office at GHS.  Copies are available upon request.  Please allow up to 48 hours to process your request.

     Additional 2021-2022 GISD Athletic Physical Forms are available in your school’s front office, coaches’ office, the GISD Athletic Department office located at 217 North Jones Street, both GHS Athletic Training Rooms, and online at www.granburyisd.org/piratesportsmedicine.
