Required Documents for All Student Athletes
The following documents are required of each student (including athletes, managers, athletic trainers, statisticians, filmers, etc.) participating in Athletics each school year. Every document must be completed in full and returned to the appropriate coach prior to the athlete participating in any practice, scrimmage, tournament, or game.
Athletic Physical Packet
Students who will participate in Granbury ISD athletic programs must complete mandatory physical examinations.
Athletic physicals are required by the University Interscholastic League (UIL) and GISD and must be completed before a student in grades 7-12 may participate in any practices or games.
Requirements for the 2020-2021 school year have been modified by the UIL in regards to athletic physicals. Due to the COVID-19 situation, any student with a current 2019-2020 school year athletic physical exam that is in good standing, will not have to get a new physical exam for the upcoming school year. However, the Medical History and the Travel Form (see GISD Physical Packet below) will still need to be completed for the 2020-2021 school year. Furthermore, if there is some indication on a student's medical history form that requires further medical evaluation, the student athlete will be required to obtain a new athletic physical.
If the student does not currently have an athletic physical exam, such as an incoming 7th grader, they will be required to complete the athletic physical exam and packet prior to any participation in practices, games, and/or events.
All parents and students have the option of completing this requirement at the medical provider of their own choice – a physician, a physician assistant licensed by the State Board of Physician Assistant Examiners, a registered nurse as an advanced practice nurse by the Board of Nurse Examiners, or a doctor of chiropractic. The district also sponsors events on campus for students to complete physicals.
Athletic trainers will collect all completed forms, which will be kept on file. Parents may request copies if needed, allowing 48-72 hours for processing.
