Subject: Accessing Your 1095 Health Coverage Tax Form
Dear Staff,
Pursuant to the Paperwork Burden Reduction Act (H.R. 3797) enacted on December 23, 2024, your 1095 Form will no longer be automatically mailed or hand delivered to you. Instead, this Form will be available to Responsible Individuals upon request, and you have several options to request this Form.
We partner with ETC Companies to prepare our 1095s and they will provide your form to you directly via email if you use their secure online form for retrieval. See below.
Via Email: Go to: theetccompanies.com – Click “ETC RETRIEVE” in the menu bar. Complete the form and submit. Your 1095 form will be delivered from the email address: [email protected] within 30 days of submission.
Via Phone: Call the HR/Benefits office if you need assistance getting the retrieve email through to ETC at (817)408-4190.

If you choose to contact us directly, you will receive your 1095 Form by the ACA furnishing deadline OR within 30 days of the request, whichever is later.
This Tax Document is significant as it indicates whether the plan offered to you and your dependents met the IRS "Minimum Value" standard and if the employee-only coverage was considered "Affordable" under IRS guidelines. The IRS continues to use this information to determine the Responsible Individual’s eligibility for premium assistance in the Marketplace.
