Student Meal Account Purchase Restrictions Request
To place a purchasing restriction on your student's meal account, please follow the link below. Restrictions will remain active on your student meal account until you contact the Child Nutrition Department at 817-408-4085 to remove the note.
Do not use this form for Allergy Restrictions. The Special Dietary Request form must be completed by the parent/guardian and student's medical doctor to accommodate a life threatening allergy.
