Health Services Consent Form
In accordance with Texas Senate Bill 12, Granbury ISD is providing this annual notice of the campus-based health services available to students. As a parent or guardian, you must complete the 2025–26 Health Services Annual Consent Form to either opt in or opt out of these services by August 31, 2025.
Campus-Based Health Services May Include
- Acute Care: Immediate care for minor illnesses and non-life-threatening injuries (e.g., temperature check, scrape cleaning, headache assessment).
- Chronic Disease Management: Support for conditions such as asthma or diabetes, including medication administration and individualized health care plans (IHPs).
- Medication Administration: Dispensing prescribed or over-the-counter medications per physician’s orders with parental consent.
- Preventive Care: Immunization reminders and health resources.
- Care Coordination: Collaboration with families, physicians, and staff to support student health needs during the school day.
- Health Education: Information for students and parents on prevention, wellness, and managing chronic conditions.
- Mental Health Support: First-aid and crisis intervention with referral to counseling services as needed.
- Addressing Health Barriers to Learning: Identifying and supporting students whose health impacts learning (including IEP/504 participation).
- Community Referrals: Connecting families to healthcare providers and social services.
If You Opt In:
The campus nurse may assess and provide care if your child becomes ill or injured, returning them to learning when possible.
If You Opt Out:
The campus nurse may not assess or treat your child except in a life-threatening emergency.
- For minor injuries (e.g., scraped knee), a parent/guardian will be contacted to determine care.
- For suspected illness, a parent/guardian must pick up the student.
