April 21, 2014

Students who will participate in Granbury ISD athletic programs during the 2014-15 school year will have an opportunity to get their mandatory physical examinations during sessions sponsored by Lake Granbury Medical Center in May 2014.

The exams will be conducted on the following dates:

Acton Middle School

Wednesday, May 7

2:00-6:00 P.M. (Grades 6-8)

Granbury Middle School

Wednesday, May 21

2:00-6:00 P.M. (Grades 6-8)

Students in grades 9-11 may also participate but will be assigned a location and time by the athletic trainers.

Granbury ISD Cost for the exam is $20 cash.

Athletic physicals are required by the University Interscholastic League (UIL) and GISD and must be completed before a student may participate in any practices or games or serve as a manager or student athletic trainer.

This event is not mandatory, and all parents and students have the option of completing this requirement at the medical provider of their own choice – a physician, a physician assistant licensed by the State Board of Physician Assistant Examiners, a registered nurse as an advanced practice nurse by the Board of Nurse Examiners, or a doctor of chiropractic.

Athletic physical packets must be completed by the parent or guardian prior to the examination. Forms are available at Granbury High School, Crossland Ninth Grade Center, AMS, GMS, both training rooms at the high school, and the GISD athletic office. The forms may also be found online on the GISD website at

Athletic trainers will collect all completed forms at the conclusion of the exams and will be kept on file. Parents may request copies if needed, allowing 48-72 hours for processing.

For more information, please contact athletic trainers Andy Rankin at (817) 408-4645 or Connie Rhodes at (817) 408-4655.
